Cold water shouldn’t hurt. If it does, your teeth are losing mineral.

GUIDE · SENSITIVE TEETH

Cold water shouldn’t hurt. If it does, your teeth are losing mineral.

The hard outer layer of your teeth is what keeps cold away from the nerve. Acid wears down that layer a little at a time, and it gets thinner over time. Once it’s thin enough, cold gets through. Most sensitivity toothpastes numb the feeling so you notice it less, but the thinning carries on underneath.

Five signs your teeth are wearing down

If cold makes you wince, that’s the first clear sign, and it counts double when it keeps coming back. It isn’t the only warning. Do you recognise yourself in any of these?

Cold drinks, ice or cold air give you a short sharp pain.

The pain stops the moment the cold is gone.

It’s several teeth, usually on one side.

You breathe through your nose in cold weather without thinking about it.

Your teeth look see-through, uneven or rough along the edges.

WHY IT HAPPENS

The pain isn’t cold reaching your nerve. It’s fluid moving.

Under the hard outer layer of your teeth there’s a softer one, and it isn’t solid. It’s full of tiny channels running down to the nerve, and every one of them is full of fluid.

While the hard layer covers them, nothing gets to them. Wear that layer thin, and the channels open up at the surface.

Then cold water lands on an open channel. The fluid inside it moves, the movement pulls on the nerve at the bottom, and you wince.

That’s why it stops the second the cold is gone. The cold itself isn’t damaging, but it’s a sign your enamel defenses are down.

So there are two things you can do about it. You can numb the nerve, so you feel less of what’s happening. Or you can plug the channels, so the cold can’t get in.

Numb the nerve and you feel less while your teeth carry on thinning. You removed the warning, but the damage is still there.

Why your tooth care routine isn’t fixing this

You’ve tried the obvious things, and none of them gets near the channels.

✕ Sensitivity toothpaste

It numbs the nerve. It doesn’t touch the channels, so the cold still gets in, you just feel less of it. Stop using it and the pain is back within about a week.

✕ Avoiding cold drinks

You feel it less because you’ve stopped setting it off. The channels are still open and the enamel is still thinning.

✕ Whitening strips

Bleaching worsens sensitivity by weakening enamel. Plenty of people got here that way.

Nothing blocks the channels. Until something does, cold will keep getting straight through.

WHERE IT STARTS

Eēsē Remineralising Toothpaste blocks the channels the cold comes through

Eēsē Remineralising Toothpaste is a complete daily toothpaste for enamel care. It contains hydroxyapatite, the mineral your enamel is made of, plus xylitol, without SLS.

For best results, use Remineralising Toothpaste alongside our Balancing Toothpaste. Balancing combines hydroxyapatite with paraprobiotics for microbiome-friendly care while you sleep.

Use Remineralising in the morning for enamel care before the day’s food and drink, and Balancing at night.

✓ It blocks the channels. Hydroxyapatite is the mineral your teeth are made of, and it’s in this toothpaste. It settles into the open channels and blocks them, so they can’t carry the cold signal to your nerve.

✓ It puts mineral back into the outer protective layer. That’s the layer that got thin and let the channels open in the first place.

✓ It cleans without stripping. There’s no SLS in it, the detergent in most toothpaste that makes a sore mouth feel worse. This one foams with something gentler and it doesn’t sting.

Brush twice a day for two minutes, then spit but don’t rinse. Rinsing washes off the thing you just put on. For a tooth that’s worse than the rest, put a little on your fingertip and dab it onto the spot.

THE EVIDENCE

Hydroxyapatite beat a placebo toothpaste by about 40%

39.5%

less sensitivity than a placebo toothpaste, across 22 clinical trials

Twenty-two clinical trials studied what people said about their own teeth. Hydroxyapatite came out 39.5% ahead of a toothpaste with nothing active in it.

Against fluoride toothpaste in the same analysis, hydroxyapatite came out 23% ahead.

And on decay, where fluoride has the strongest evidence in dentistry, hydroxyapatite matches it. In an 18-month trial of 194 adults, a 10% hydroxyapatite toothpaste performed just as well as a 1,450 ppm fluoride one. 89.3% of the hydroxyapatite group finished with no new decay. On fluoride it was 87.4%.

The difference is the warnings on the labels. Fluoride toothpaste comes with dosing rules and ingestion warnings: a smear for under-threes, a pea from three to six, spit and don’t swallow, watch them closely while they brush. Those rules exist because swallowing too much fluoride while adult teeth are still forming can mark them for life, and high amounts can have systemic health risks. Hydroxyapatite is the mineral your teeth are already made of, so it doesn’t need an instruction sheet.

WHAT TO EXPECT

Two weeks, four weeks, eight weeks

Hydroxyapatite doesn’t numb anything, so there’s nothing to feel on day one. It builds up on the surface, and that takes a consistent routine.

1 · Two weeks

Mineral is going on twice a day and staying there, because you’re spitting rather than rinsing it away.

✓ You’ve stopped rinsing after brushing.

✓ You’re dabbing a little onto the worst tooth with a fingertip.

2 · Four weeks

At this point of a consistent routine, you’ll be noticing the results and enjoying things you’d forgotten.

✓ Cold air on a walk.

✓ A cold drink you didn’t brace for.

3 · Eight weeks

Eight weeks is where most trials mark the finish line, but it’s just the beginning. Keep going and the surface keeps building.

Questions before you switch

How is this different from the sensitivity toothpaste I’m using?

Yours probably works on the nerve and simply numbs the feeling. This one plugs the channels the cold comes through.

How long until cold water stops hurting?

Most trials measure at two, four, six and eight weeks. Expect it to ease off gradually across that time rather than switching off overnight.

Does hydroxyapatite actually work, or is it wellness marketing?

It works. Its use was developed by NASA and it’s already common in toothpaste in Japan. It’s the mineral your teeth are made of, and across 22 clinical trials it came out 39.5% ahead of a placebo toothpaste on sensitivity.

No fluoride. Isn’t that the part that works?

Fluoride works. So does this. An 18-month trial found that a hydroxyapatite toothpaste performed just as well as a fluoride one on new decay, and it comes without the dosing rules.

Can I use it at night too?

Yes, and you should. Twice a day is the instruction.

Can children use it?

Yes. There’s no fluoride in it to worry about swallowing.

Will it whiten my teeth?

It polishes surface stains off, so your teeth look brighter. It doesn’t bleach, and that matters here, because bleaching is one of the things that makes teeth more sensitive.

Does it sting?

No. There’s no synthetic menthol or SLS in it, which is what makes most toothpaste sting and leaves your mouth feeling stripped. It still foams and tastes of mint.
